Community Based Services

Community Transport is designed to assist frail/ aged or disabled people, and people who are unable to use other forms of transport, to access medical and therapy appointments. The program provides information, support and training to volunteers who wish to participate. The service is run on an as required basis and is available Monday-Friday.

People who can use the service are:

• Frail and/or aged and/or their carers
• People with disabilities and/or their carers
• Family with individuals on low income requiring medical or therapy treatment in Albury or Wodonga

Transport will be provided if clients or groups have no alternative transport arrangements available to them including public or private transport. The Community Vehicle does not duplicate the role of the Ambulance Service. There is a cost to the community vehicle. The fee is able to be claimed back again by submitting a claim to the Victorian Patient Transport Assistance Scheme.

The Day Activity Centre provides a range of indoor/ outdoor activities and opportunities for those community members who require companionship and friendship. Activities include exercises, craft, book club, brain gym, movies, bowls and music. The Centre also promotes good mental health, physical strength, and self-confidence, encompassing both carer and client while ensuring longevity in one’s home.The program is flexible and able to meet individual needs and a nourishing hot meal is included. This individualised approach ensures quality of life and general well-being is maintained. The Centre is open four days a week and its clients are well supported by a dedicated team of caring staff and volunteers.

Corryong Health adopts the active service model, which is helping people stay active, involved and doing as much for themselves as possible. This is proven to help people stay more independent with a greater sense of wellbeing.

Domiciliary Midwifery nursing provides in-home support for mother, baby and the family following discharge from hospital. We also provide low risk clients with ante natal care.

The Home and Community Care Program for Younger People (HACC PYP) supports Victorians from birth to 65 years, and Aboriginal and Torres Strait Islander people from birth to 50 years if their capacity for independent living is at risk. This may be due to chronic illness, mental health issues, disability or other conditions where they need one-off, intermittent or ongoing support to undertake the activities of daily living.

HACC PYP supports Victorians to optimise their health and wellbeing, while maintaining or regaining their independence to live safely in their homes, and actively participate in their community.

Corryong Health is registered with the HACC PYP and funded to provide the  following services:

• Domestic Assistance
• Lawn Maintenance
• District Nursing

Home delivered meals are available 5 days a week with frozen meals being available on the weekend. People needing meals will need to be assessed for eligibility by My Aged Care.

Meals are delivered by Community Volunteers Monday to Friday for those who live in Corryong.  For those who live out of town, frozen meals are available to be picked up weekly by clients or family members.

It is important to note that during the bushfire season or on Catastrophic Fire Danger rating (Code Red) days, some services may change timings or not be available.

The NDIS is an insurance scheme that assists people with a permanent and significant disability to access mainstream and community supports and services. The NDIS, funds reasonable and necessary supports, including Early Childhood, Early Intervention (ECEI) supports, for its participants. Participants are people who have a permanent and significant physical or psychosocial disability which has substantial impact on daily life and have proven eligible for the NDIS.

Corryong Health is registered with the NDIS and funded to provide a number of following ‘support’ types:
