INTAKE OFFICER

Location: Corryong VIC, Australia
Job Type: Part Time, 6 days per fortnight

About the Role:

The Intake Officer works with clients, families, care coordinators, My Aged Care portal, Internal and External service providers to streamline service provision to achieve the best outcome for the Upper Murray community. The Intake Officer will utilise their clinical knowledge to assist clients in identifying the appropriate Allied Health, Mental Health, Medical Services or Community-based services they require to meet their needs and achieve their goals.

Requirements:

The Intake Officer is open to an experienced Physiotherapist, Occupational Therapist or Registered Nurse. As an Intake Officer, you will need to have knowledge of:

  • Care Pathways
  • Financial Pathways
  • Experience in complex clients.

As part of the Financial Pathways component, you are required to have knowledge of:

  • NDIS and PYP
  • Home Care Packages
  • TAC and Workcover
  • CHSP
  • Small Rural Funding
  • Medicare and Fee for Service
  • GP Management Plans

Corryong Health (CH) is a public sector agency and one of six multi-purpose services in Victoria. CH is funded by Commonwealth and Victorian State Government funding, and provides a broad range of hospital, residential aged care, medical and primary health and community support services. We provide services to the Corryong community and operate a Medical Clinic, Health and Fitness Centre and Retirement Village.
